The undersigned hereby acknowledges that the undersigned participated in the Film produced by EU Films as part of the Film Workshop class at Evangel University.

VIDEO SHOOT RELEASE: The undersigned hereby irrevocably grants to EU Films and its representatives, and to Evangel University, its licensees, agents, successors and assigns, the right, but not the obligation, in perpetuity throughout the world and in all media, now or hereafter known, to use (in any manner EU Films and Evangel University deem appropriate, and without limitation) in and in connection with the Film, by whatever means exhibited, advertised or exploited: the undersigned’s appearance in the Film, still photographs of the undersigned, recordings of the undersigned’s voice taken or made, any music sung or played by the undersigned, and the undersigned’s actual or fictitious name.

Furthermore, the undersigned acknowledges that Evangel University or its assigns may shoot behind the scenes “making of the (the Film)” footage, for use in a motion picture, and the undersigned hereby irrevocably grants to EU Films and Evangel University, its licensees, agents, successors and assigns, the right, but not the obligation, in perpetuity throughout the world and in all media, now or hereafter known, to use (in any manner it deems appropriate, and without limitation) in and in connection with the motion picture, by whatever means exhibited, advertised or exploited: the undersigned’s appearance in the motion picture, still photographs of the undersigned, recordings of the undersigned’s voice taken or made, any music sung or played by the undersigned, and the undersigned’s actual or fictitious name.

LIMITED LIABILITY RELEASE: Furthermore, the undersigned understands that during the undersigned’s participation in the Film Workshop project, the undersigned may be exposed to a variety of hazards and risks, foreseen and unforeseen, that may include, but are not limited to, personal injury, property damage and death. To the fullest extent allowed by law, the undersigned agrees to waive, discharge claims and release from liability the Film Workshop, EU Films, and Evangel University, its officers, directors, employees, agents and leaders from any and all liability on account of, or in any way resulting from injuries or damages. The undersigned understands and intends that this assumption of risk and release is binding upon the undersigned’s heirs, executors, administrators and assigns, and includes any minors accompanying the undersigned on the Film project.
